Findings
According to documentation, on at least two (2) occasions, the Alleged Victim (AV) received additional doses of a prn antianxiety medication, which is outside the parameters of the physician order. There was no negative outcome to the AV. The facility failed to follow medication physician orders, which is a violation of Oregon Administrative Rules.
