Findings
On or about November 16, 2022, the Alleged Victim (AV) was found with unexplained bruising on his/her left rear cheek and thigh. AV had a history of being found on the floor. The facilty failed to provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
