Findings
On or about November 21, 2022, Alleged Perpetrator #2 (AP2) was assisting the Alleged Victim (AV) with dressing. During the process of dressing AV, AP2 yanked AV's arm, causing pain and unreasonable discomfort to AV. AP2's actions are a violation of resident rights, are considered neglect of care and constitute physical abuse. The facility failed to protect AV from physical abuse, which is a violation of Oregon Administrative Rules.
