Findings
Alleged Perpetrator 2 (AP2) failed to provide wound care to Alleged Victim (AV) . Documentation indicates AP2 is responsible for direct care, and treatments to residents. AV was to receive wound care to h/h pressure wounds. Documentation indicates from approximately September 21, 2022, to October 04,2022, AV did not receive wound care as ordered, resulting in AV becoming upset and caused a risk of serious harm. AP2's actions are considered neglect of care and constitutes abuse. The facility failed to provide wound care which is a violation of Oregon Administrative Rules.
