Findings
The facility failed to provide a safe medication administration system. On or about November 22, 2022, AV was administered a medication not prescribed to him/her. AV was sent to the hospital for observation and experienced side effects due to the medication error. The facility’s failure is a violation of resident rights, is neglect of care and constitutes abuse. The allegation that Alleged Perpetrator 2 (AP2) failed to provide appropriate services was investigated and determined no abuse occurred.
