- Alleged Victim (AV) service plan indicates AV has a history of wandering and getting lost causing anxiety. AV takes medication at night for increased anxiety at bedtime. Staff will add non-pharm interventions and document if successful or not if onset of behavior occur. Interventions for AV's behavior for feeling overwhelmed, anxious, fearful, angry, sullen, crying include; Staff to offer AV job around the community to complete, Staff offer to take AV on a walk, Staff to offer a snack/drink with 1:1 conversation and Engage AV in activities. From approximately September 16, 2022-November 17, 20222, AV showed increased agitation and anxiety by exit seeking and yelling. On or about October 20, 2022, AV started antibiotics for an UTI. Even after AV finished antibiotics, AV continued to be agitated and exit seek. On or about November 17, 2022, AV woke up agitated and violent towards staff. AV started to throw items resulting in skin tear/cuts on the left leg. The facility failed to adjust the service plan and implement reasonable interventions for AV leading to continued aggressive behavior, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
