The facility failed to provide services as ordered by Home Health according to the Alleged Victim’s (AV) care needs. The failure resulted in AV’s sustaining a bruise on the right side of AV's forehead causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
