Findings
The Alleged Victim (AV) has a history of falls and requires two staff members for all transfers. On or about November 25, 2022, AV suffered an unwitnessed fall where he/she received a laceration and a bruise above his/her right eyebrow. It is suspected that AV tried to self transfer by using a table as leverage. While there are ISP's in place to assist in preventing AV's falls, it appears these are not effective and AV continues to have falls. The facility's failure to properly care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
