Findings
According to an investigation, on or about November 28, 2022, Alleged Perpetrator 2 (AP2) gave AV an incorrect dosage of his/her diabetic medication, which resulted in AV experiencing unstable blood sugar, leaving AV at risk for serious harm.   The facility and AP2 failed to provide a safe medication administration system, which is a violation of resident rights, is neglect of care and constitutes abuse.
