Findings
The facility failed to appropriate care plan and provide services according to the Alleged Victim (AV)'s needs, relating to falls and lack of appropriate interventions. Based on an investigation, AV experienced approximately seven unwitnessed non-injury falls between November 11, 2022, and November 27, 2022. On or about November 28, 2022, AV experienced another unwitnessed fall, which resulted in AV sustaining an abrasion to the right side of his/her chin. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
