Findings
The Alleged Victim (AV) relies on the facility for care. AV’s care plan states facility staff will assist AV with catheter care. On or about November 25, 2022, AV was observed to be leaking urine down his or her leg due to a missing catheter bag and staff were not aware the bag was not in place. Then on or about November 29, 2022, AV was found to have his or her catheter bag incorrectly attached to the catheter resulting in AV having no urine output and causing AV to be in unnecessary pain. AV had to be transferred to the hospital for treatment. The facility failed to follow AV’s care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
