Findings
According to an investigation, it was determined that Alleged Perpetrator 2 (AP2) pushed the Alleged Victim (AV) in the back , which caused AV emotional distress. AP2’s actions are a violation of resident rights, are considered neglect of care and constitutes physical abuse. The facility failed to provide a safe environment which violates Oregon Administrative Rules.
