Findings
The facility failed to develop and/or maintain a comprehensive care plan for The Alleged Victim's (AV) when AV required increased assistance with his/her Activities of Daily Living (ADL's) causing undesirable weight loss. Based on facility documentation, AV lost almost thirty (30) pounds between February 25, 2022, and September 27, 2022, causing AV unreasonable discomfort which is a violation of resident rights, is considered neglect of care and constitutes abuse.
