Findings
On or about 12/03/2022, Witness #3 (W3) approached Alleged Victim (AV) room and found Alleged Perpetrator 2 (AP2) being load and verbally aggressive with AV while providing care. W3 attempted to diffuse the situation but AP2 continued to yell at AV, resulting in AV becoming upset, which is considered verbal/emotional abuse. The facility failed to protect AV from verbal/emotional abuse which is a violation of Oregon Administrative Rules.
