Findings
Alleged Victim (AV) is care planned for two (2) people assist for personal care, ADL, and ambulation, mobility needs. On or about 12/03/2022, Alleged Perpetrator 2 (AP2) was providing care and repositioning AV without Assistance, resulting in unreasonable discomfort, and placed AV at risk for harm. AP2 failed to provide a safe environment and follow the care plan, which is a violation of resident rights is considered neglect of care and constitutes abuse. The facility failed to provide a safe environment which is a violation of Oregon Administrative Rules.
