Findings
According to documentation the facility failed to follow the Alleged Victim’s (AV) care plan. AV care planned to be checked every two hours. According to documentation AV experienced a fall on or about December 06, 2022. The failure resulted in AV being sent to the hospital for treatment and experiencing pain and unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
