- Amended on May 18, 2026, and re-issued to AP2:
Alleged Perpetrator 2 (AP2) notified ODHS that AP2 did not receive the initial LOD mailed on April 25, 2023. AP2 confirmed that the LOD had been sent to a previous work address rather than AP2's home address. ODHS reviewed AP2’s request and determined that AP2 did not receive a valid notice of the substantiated abuse finding. Therefore, an amended notice with the correct address was mailed to AP2.
Alleged Perpetrator 2 (AP2) failed to put sufficient interventions in place to protect Alleged Victim (AV) from Witness 7's (W7) inappropriate sexual advancements which caused a non-consensual sexual relationship between AV and W7 to occur. AV shared with AP2 that he/she felt coerced into having a sexual relationship with W7 and AP2 failed to immediately implement interventions to protect AV from W7. AP2's failure to provide a safe environment for AV is considered neglect of care which constitutes abuse. The facility failed to provide a safe environment for AV which a violation of Oregon Administrative Rules.
