Findings
On or about November 24, 2022, AV was not administered h/h medication as ordered. On or about December 04, 2022, AV was administered h/h medication late. The facility failed to provide a safe medication administration system which is a violation of Oregon Administrative Rules. This is the facility's second violation of this kind within a six-month time frame. Further rule violation(s) may result if civil penalties or further corrective action.
