Findings
The Alleged Victim (AV) was a known fall risk and had a history of multiple falls, with and without injury. On or about December 8, 2022, AV suffered a fall which resulted in bleeding from his/her nose and lip. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
