Findings
The facility failed to provide appropriate services for Alleged Victim (AV) according to his/her needs for assistance with nail care which resulted in AV getting a scratch on his/her abdomen. Based on interviews and facility documentation, AV did not receive proper nail care for over a year prior to incident which left AV’s nails long and sharp. The facility's failure to follow AV's care plan caused unreasonable discomfort which is a violation of resident rights, is considered neglect of care and constitutes abuse.
