- The Alleged Victim (AV) was scheduled to receive a pain patch every three days. Staff were to remove the old patch when applying the new patch. Hospice had advised staff to put the patch on different areas of AV's skin to assist with absorption of the medication. On or about December 3, 2022, AV's family member found two pain patches on AV at the same time, placing AV at risk for harm. The facility's failure to provide a safe medication administration system is a violation of resident rights, is considered neglect of care and constitutes abuse.
