- Alleged Victim (AV) is known to be a high fall risk prior to moving to the facility. Care plan states staff are to provide support by assisting stand by for transfer while AV is in the apartment, providing frequent safety checks, at least four times per shift. Between September 18th, through December 14th, 2022, AV suffered 10 falls, most of which led to AV sustaining injury such as laceration to the hand, bruising on left eye, bleeding from the face, skin tear to the elbows, and redness to the hip. The facility failed to provide a safe environment for AV, due to the repeated falls with injuries for an extended period of time, which is a violation of resident rights, is neglect of care and constitutes abuse.
