Findings
The facility failed to follow Alleged Victim (AV) care plan. AV is care planned for showers two (2) times weekly, staff to check AV skin with bath/shower and report any reddened open areas. Staff, trim fingernails, and toenails, and AV will be assisted with toenail and fingernail care after each bath/shower. Documentation provided to the Department indicates AV received two (2) showers approximately in the month of November, and one (1) shower approximately in the month of December. Documentation indicates AV left 4th digit toenail coming off on approximately November 21, 2022, and photos from approximately December 2022 show poor condition of AV toenails and skin condition of feet, resulting in AV complaining of pain, and repeated unreasonable discomfort. The facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
