Findings
The facility failed to provide appropriate services according to the Alleged Victim’s (AV) care needs for assistance with personal hygiene. Based on facility documentation and interviews, facility staff were inconsistent with following AV's care plan to assist with hygiene care which resulted in AV experiencing unreasonable discomfort in the form of a red, itchy, bumpy rash. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
