Findings
On or about June 23, 2022, the Alleged Victim (AV) received a package at the facility, which was signed for by facility staff, but was not delivered to AV. AV's package went missing. AV's package was taken by an unknown individual, Alleged Perpetrator #2 (AP2). AP2's actions are a violation of resident rights, are considered neglect of care and constitute financial abuse. The facility failed to have policies and procedures in place to ensure the safety of resident belongings, which is a violation of resident rights, is considered neglect of care and constitute abuse.
