- Witness 1 (W1) has a history of aggression and altercations with staff and other residents resulting in harm, and injury. Documentation provided to the Department indicates W1s interventions put into place after prior altercations have been monitor and redirection. On or about December 11, 2022, W1 walked by Alleged Victim (AV) and pushed AV’s head back, resulting in AV becoming visibly upset. The facility failed to appropriately care plan and implement reasonable interventions to address W1s ongoing aggression and behaviors, which is violation of resident rights, is considered neglect of care and constitutes abuse.
