Findings
The Alleged Victim (AV) was a known fall risk and had suffered several falls between September 28, 2022 and December 7, 2022. The facility did not implement any interventions to reduce the number of Av's falls during this time. AV suffered a fall on or about December 20, 2022, which resulted in a fractured arm and facial trauma. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
