- The facility failed to provide appropriate services according to Alleged Victim (AV)'s needs, related to a fall that resulted in an injury. Based on an investigation, on or about November 4, 2022, AV experienced a fall that required AV to use a sling. The sling did not fit properly which caused AV to wear the sling incorrectly or not at all, resulting in unreasonable discomfort and the risk of additional complications. The facility did not contact AV’s physician and AV was not provided with a new sling until the follow-up visit on November 18, 2022. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
