Findings
According to documentation, Alleged Perpetrator 2 (AP2) failed to provide a safe medication administration system. The failure resulted in the Alleged Victim (AV) receiving another resident’s medication, experienced a fall when ambulating and went to the hospital for evaluation, which is a violation of resident rights and considered neglect. The facility failed to provide a safe medication administration system which is a violation of Oregon Administrative Rules.
