Findings
The Alleged Victim (AV) relies on the facility to manage his/her medications. According to an investigation, on or about December 20, 2022, the facility failed to provide a safe medication administration system for the AV. There was no observable and/or documented evidence of a negative outcome. The failure is a violation of Oregon Administrative Rules.
