Findings
On or about December 30, 2022, staff heard screaming coming from the Alleged Victim's (AV) room. Staff went to the room and observed Alleged Perpetrator #2 (AP2) standing over AV, yelling at him/her in an unreasonable manner. AP2's actions are a violation of resident rights, are considered neglect of care and constitute verbal abuse. The facility's failure is a violation of Oregon Administrative Rules.
