Findings
According to the documentation, the facility failed to follow the care plan to provide 1:1 care for the Alleged Victim (AV) to prevent falls. The failure resulted in the AV suffering two falls on or about December 31, 2022, one with injury, which is violation of resident rights, is considered neglect of care and constitutes abuse.
