Findings
Witness #1 (W1) is known to become agitated and verbally abusive to residents in the facility and is care planned to be removed to a private area to allow W1 to have personal space to reduce anxiety and acting out. W1 entered the dining area and was verbally abusive to the Alleged Victim (AV), swearing at him/her, causing AV unreasonable discomfort. After this incident, staff took W1 to a public area while W1 was still agitated. The facility failed to follow W1's care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
