Findings
On or about December 30, 2022, the Alleged Victim (AV) engaged in a verbal altercation in the dining room with Witness #1 (W1), which caused AV to be visibly upset. AV was asked to remove him/herself from the dining room and go to his/her room or to the common area. AV is care planned to be re-directed to a quiet area when in an agitated mood. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
