- Alleged Victim (AV) relies on the facility for his/her care. AV was found on the floor by a staff in AV’s bedroom without any clothing on. When the staff went to find Alleged Perpetrator 2 (AP2) and Alleged Perpetrator 3 (AP3) on duty in the memory care unit, they were outside in the courtyard smoking. AP2 and AP3 had been found outside twice that night together. AP2 and AP3 had been told they could not leave the floor together as it is considered abandonment of a resident. AP2 and AP3 failed to provide a safe environment for AV, which is a neglect of care and constitutes abuse. The facility failed to assure AV received a safe environment, which is a violation of Oregon Administrative Rules.
