Findings
According to the documentation, the facility failed to follow the care plan to attach the tab alarm to the Alleged Victim (AV) while they are in bed. The failure resulted in the AV rolling out of bed and landing on their crash pad putting them at risk for injury, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
