Findings
Alleged Victim’s (AV’s) care plan states he/she is not a fall risk. Between October 25, 2022, through January 3, 2023, AV sustained five (5) falls, some resulting in injury. On or about January 4, 2023, AV was found to have a large bruise to his/her lower back. The bruise is believed to have been caused by the fall the day before. The facility failed to provide a homelike environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
