Findings
On or about January 1, 2023, the Alleged Victim (AV) received incorrect medication from staff. AV showed signs of low blood pressure and was sent to the hospital for evaluation. AV returned to the facility with instructions to eat and drink something if AV became light headed. The facility's failure to provide a safe medication administration system is a violation of resident rights, is considered neglect of care and constitutes abuse.
