- Alleged Victim (AV) has history of aggressive/verbal behaviors and entering other residents’ rooms resulting in physical altercations with residents and staff. AV and W1 have history of prior verbal altercation with threats to punch each other after AV entered W1 room on December 3, 2022. AV is service planned for staff to redirect as an intervention, however, documentation provided to the department indicates AV does not respond to redirection. On or about January 5, 2023, AV knocked on W1 door and pushed h/h way into W1 room. W1 punched AV causing AV to fall, hit h/h head on a bench and was knocked out. AV was not talking or moving to any stimuli, and sustained red mark on chin, head, and an abrasion on h/h leg. AV was transported to the emergency room for medical attention. The facility failed to appropriately adjust AV care plan and implement reasonable interventions to address AVs ongoing behaviors and entering other residents’ rooms, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
