Findings
On or about January 6, 2023, Witness #1 (W1) punched the Alleged Victim (AV) in the stomach, causing unreasonable discomfort. W1 and AV have a history of altercations and there is a service plan in place to keep these two away from each other. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
