Findings
The facility failed to implement interventions or appropriately care plan for the Alleged Victim’s change of condition and known fall risk. This failure resulted in AV falling on or about January 2, 2023, resulting in bruising and a skin tear. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
