Findings
According to an investigation, on or about January 13, 2023, the facility failed to administer medication as ordered for the Alleged Victim, placing AV at risk for harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The allegation that the Alleged Perpetrator 2 (AP2) failed to provide a safe medication administration was investigated and determined to be not substantiated.
