Findings
The Alleged Victim (AV) has a seizure disorder and is known to have falls in the past. On or about January 15. 2023, AV fell and was found on the ground. The facility failed to appropriately care plan regarding AV's falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
