Findings
According to the documentation, the facility failed to follow the care plan for the Alleged Victim (AV) and Witness #1 (W1) by not monitoring them when they are together. The failure resulted in W1 touching AV in a sexual manner causing emotional harm. The facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
