- The facility failed to provide a safe medication administration system to ensure the Alleged Victim’s (AV) pain medication was administered as ordered. According to documentation On or about January 28,2023 and January 29, 2023, AV was administered a higher dose of pain medication resulting in AV showing symptoms hallucinations and experiencing unreasonable discomfort. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse. According to documentation, the Alleged Perpetrator (AP2) administered a higher quantity then prescribed On or about January 28, 2023. AP2’s actions are considered neglect of care and constitutes abuse, which is a violation of Oregon Administrative Rules. According to documentation, the Alleged Perpetrator (AP3) administered a higher quantity then prescribed On or about January 28, 2023. AP3's actions are considered neglect of care and constitutes abuse, which is a violation of Oregon Administrative Rules.
