Findings
On or about January 20, 2023, Alleged Perpetrator 2 (AP2) left Alleged Victim (AV) unattended in the shower to help another resident. AV is care planned for a one-person assist with showering/bathing due to his/her fall risk. Based on interviews and facility documentation, the facility admitted to not informing AP2 to never leave AV unaccompanied in the shower. The facility's failure to properly train staff on AV's care plan, placed AV at risk of harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse. AP2 allegedly neglect AV. An investigation determined no abuse occurred by AP2.
