Findings: Alleged Victim (AV) relies on the facility for his/her care. AV is care planned to have skin condition monitoring for new concerns on shower days, which is two times per week. On or about February 1, 2023, AV was found to have toenails which had grown over the edge of h/h toe and were embedding into the flesh of AV's toes and required licensed staff intervention. The facility failed to follow the care plan which is a violation of resident’s rights is neglect of care and constitutes abuse.
Sanction: RCFCP24-00933 $1125.00 fine assessed
