Findings
The Alleged Victim (AV) was serviced planned to have assistance with showers 4 days per week. AV has gone without showers multiple days, causing him/her to experience body odor, dry skin, itchy scalp and low self esteem, which caused AV unreasonable discomfort. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
