Findings
According to the documentation, the facility failed to follow Witness 1’s (W1) care plan to prevent resident to resident altercations. The failure resulted in W1 approaching the Alleged Victim (AV) and pinching the top of their hand causing the AV to yelp in pain, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
