- The Alleged Victim (AV) relies on the facility for his or her safety. The facility has no procedure for alerting staff of new residents’ arrivals except through word of mouth and no clear way to differentiate between visitors and residents. On or about February 15, 2023, AV moved into the facility, and on February 16, 2023, AV followed Alleged Perpetrator #2 (AP2) out a back door and stopped, AV told AP2 s/he forgot the code for the second door that leads outside the facility. AP2 “punched” in the code and AV walked outside. AV was found out front of the building approximately ten (10) minutes later. The facility parking lot is across from a highway. The facility failed to provide a safe environment, which is a violation of resident rights is considered neglect of care and constitutes abuse. AP2 did not receive information about AV being a new resident and did not recognize AV as a resident. The Allegation that AP2 failed to provide a safe environment was investigated and the determination was not substantiated.
